We welcome three fantastic speakers from across the nations – Lyndsey Dunn, Lucy Lewis and Stacey Finlay to present on different areas of frailty and how we can support older adults in our roles. There will be time for any questions you may have, and a CPD certificate will be available following attendance to the webinar.

Lyndsey Dunn – ‘"Discharge Without Delay" initiative, aimed at enhancing timely hospital discharges for frail older patients in Scotland. We will explore the current challenges in service delivery, propose a multidisciplinary community care model that prioritizes early intervention and integrated support, and outline future goals for improving care coordination between healthcare providers. The initiative's alignment with broader healthcare strategies will also be highlighted.
Lyndsey Dunn is a dedicated Clinical Service Manager with over 22 years of experience in NHS Scotland, currently overseeing community flow, integrated discharge teams, and specialist services at Fife Health and Social Care Partnership. With a strong commitment to enhancing patient outcomes, she specializes in Frailty and person-centred discharge planning.
Lyndsey holds a Bachelor of Science in Adult Nursing from Edinburgh Napier University and an MSc in Gerontology and Global Ageing from Stirling University. Her career has included pivotal roles such as Frailty Nurse in Acute Medicine and POPS Specialist Nurse, where she focused on the care of older adults and the management of patients at risk of delirium.
As the Lead Nurse for Quality Improvement and Standards, Lyndsey was instrumental in developing policies that elevate service delivery and patient care. She is a prominent advocate for innovative healthcare solutions, sharing her insights at prestigious national events and webinars. Her contributions include significant input into the BGS report "Reablement, Rehabilitation, Recovery: Everyone’s Business," which reflects her dedication to advancing healthcare practices.
Currently serving as Chair of the BGS Nurses and AHPs Council, Lyndsey continues to champion integrated healthcare approaches, making her a respected leader in nursing and healthcare management.
Lucy Lewis - “How can I in my role support older people across the frailty pathway?”
Lucy Lewis is a Consultant Nurse specialising in frailty and older people and works in a Hospital at Home service in Wiltshire. Following completion of her MSc in Advanced Clinical Practice in 2015, Lucy undertook the Health Education England Frailty Quality Improvement fellowship before becoming a Trainee Consultant on the HEE South-East Consultant Practitioner Development Programme (2017-2020). She has worked at consultant level practice ever since. Lucy is undertaking a part time PhD focusing on How older Individuals & clinicians make decisions about cancer treatment and Support (The CHOICES study). She has represented Nurses and AHPs (NAHP) on the British Geriatrics Society Policy & Communications committee and is now the co-chair of the BGS Frailty & Urgent Care Special Interest Group committee having been the NAHP council chair from 2019-2021. Lucy is a Florence Nightingale Foundation Alumni Champion having completed her scholarship in 2019, traveling to Scotland Canada to explore examples of government supported initiatives which actively sought to improve integrated pathways for older people and those living with frailty. She is an Honorary Assistant Professor in Frailty and Leadership at Coventry University.
Stacey Finlay – ‘Fallproofing Frailty: A Balancing Act in Prevention’
Stacey Finlay is an adult nurse specialising in the care of older people and currently works as a Deputy Manager with Healthcare Ireland. She holds multiple postgraduate qualifications in advanced practice and gerontology, and is an Honorary Lecturer at Queen’s University Belfast. Stacey represents Northern Ireland on the British Geriatrics Society Nurse & AHP Council, and is Deputy Lead of the ÂÌñ»»ÆÞ Independent Sector Network NI. She is passionate about frailty education, interdisciplinary collaboration, inspiring the next generation of older people's nurses and improving outcomes for older adults living in any and every setting.
